Petition Introduction of 'British Standard's' to NHS Health Care

I want to see the introduction of a standardised test which patients would receive every time they visited the Doctors, regardless of the reason the appointment was made.
The aim of which would be to improve patient record's as well as patient/doctor relationship's.
This also would help to highlight and allow for the identification of any problem's that occur over long periods of time.
